This guide is general education that summarizes published research and guidance from groups like the American Academy of Pediatrics, the CDC, and the WHO. It isn’t medical advice, and every child develops on their own timeline. Your pediatrician knows your child; if something feels wrong, call them. In an emergency, call 911.

The 2-month shots were a day or two ago. Today is for the sore leg, the extra sleep, and the one rule that differs at this age.

## Heart

Hold them more than usual. A Cochrane review of 10 trials (1,066 babies) found that breastfeeding during vaccination cut crying by about 38 seconds ([Harrison et al., 2016](https://pubmed.ncbi.nlm.nih.gov/27792244/)), and a 2026 guideline strongly recommends feeding or sucking for comfort at shot time ([Taddio et al., 2026](https://pubmed.ncbi.nlm.nih.gov/42767684/)). The shots are done, but your arms and a feed are still the best you have for a sore day.

## Head

Soreness, redness, and swelling at the shot site usually start within a day and last 3 to 5 days, up to 7 with DTaP. Fussiness, restless sleep, extra sleep, and eating a little less usually pass within 24 to 48 hours. A warm, wet washcloth on the spot for 10 minutes and gentle massage can help ([AAP](https://www.healthychildren.org/English/tips-tools/symptom-checker/Pages/symptomviewer.aspx?symptom=Immunization+Reactions)).

## Body

Fever is the exception at this age. Under 12 weeks, the AAP's guide says any fever means call your pediatrician now, and don't give fever medicine before your baby is seen ([AAP](https://www.healthychildren.org/English/tips-tools/symptom-checker/Pages/symptomviewer.aspx?symptom=Immunization+Reactions)). A rectal reading of 100.4°F (38°C) or higher counts ([AAP](https://www.healthychildren.org/English/health-issues/conditions/fever/Pages/Fever-and-Your-Baby.aspx)). About 1 in 20,000 to 1 in 100,000 US babies who get rotavirus vaccine may develop intussusception, a bowel blockage, within a week ([CDC](https://www.cdc.gov/vaccine-safety/vaccines/rotavirus.html)). Call now for vomiting or severe crying after that vaccine, or for nonstop crying that lasts more than 3 hours after any shot ([AAP](https://www.healthychildren.org/English/tips-tools/symptom-checker/Pages/symptomviewer.aspx?symptom=Immunization+Reactions)).

## Try today

### Warm cloth and a feed

- **Time:** 10 minutes, whenever the leg seems to bother them
- **From the guide:** [After the 2-month shots](https://chorevia.com/learn/2-months#parenting-techniques)

1. Hold your baby close; feeding and sucking are among the best-supported comforts ([Harrison et al., 2016](https://pubmed.ncbi.nlm.nih.gov/27792244/); [Taddio et al., 2026](https://pubmed.ncbi.nlm.nih.gov/42767684/)).
2. Offer a feed, or a pacifier if they take one.
3. Run a washcloth under warm water, wring it out, and rest it on the sore spot for 10 minutes ([AAP](https://www.healthychildren.org/English/tips-tools/symptom-checker/Pages/symptomviewer.aspx?symptom=Immunization+Reactions)).
4. Rub the area gently, then let them doze if they want to.

## Watch for

- **Normal:** a red, slightly swollen spot for 3 to 5 days. **Call your pediatrician right away** for any fever before 12 weeks, including after vaccines ([AAP](https://www.healthychildren.org/English/tips-tools/symptom-checker/Pages/symptomviewer.aspx?symptom=Immunization+Reactions)).
- **Normal:** extra sleep and less interest in eating for 24 to 48 hours. **Call your pediatrician right away** for vomiting or severe crying after rotavirus vaccine, or nonstop crying for more than 3 hours after any shot ([AAP](https://www.healthychildren.org/English/tips-tools/symptom-checker/Pages/symptomviewer.aspx?symptom=Immunization+Reactions)).
- **Normal:** a fussier baby than usual. **Call 911** if your baby can't be woken up, or has trouble breathing or swallowing after a shot ([AAP](https://www.healthychildren.org/English/tips-tools/symptom-checker/Pages/symptomviewer.aspx?symptom=Immunization+Reactions)).

## For you

The medicine rules for your baby mean a call, not the cabinet. The AAP says not to give acetaminophen (Tylenol) to children under 2 without a doctor's guidance ([AAP](https://www.healthychildren.org/English/safety-prevention/at-home/medication-safety/Pages/Acetaminophen-for-Fever-and-Pain.aspx)), or ibuprofen before 6 months unless your doctor says to ([AAP](https://www.healthychildren.org/English/safety-prevention/at-home/medication-safety/Pages/Ibuprofen-for-Fever-and-Pain.aspx)). For the 4-month shots, too, skip medicine beforehand: in two trials with 459 babies, preventive acetaminophen at vaccination cut fevers but lowered antibody responses to several vaccines ([Prymula et al., 2009](https://pubmed.ncbi.nlm.nih.gov/19837254/)).

Your own cabinet is different. After a C-section, most people need pain medicine for a few days to 2 weeks ([MedlinePlus](https://medlineplus.gov/ency/patientinstructions/000624.htm)); if you still do at nine weeks, that's its own call; ask what's safe while breastfeeding.

If you're the partner: the thermometer and the washcloth are yours tonight. At 100.4°F or higher, you make the call.
